Since this ministry event is sponsored by two organizations - WORD FM Radio and Life Builders - you will need to fill out registration information for both organizations. All five pages which you need to fill out are located here.
When submitting paperwork, we are ok if you print on both sides of the paper.

Please complete all forms and hand in to your group leader.

Make Checks payable to your group so one check can be sent to Life Builders.
