How to Register Your Group
Important Dates
Once you have been notified that space is reserved by a $100 non-refundable, transferable to another year if necessary, deposit, you will want to begin to plan to make a first payment for each person you hope to have attend Life Builders Youth Work Camp by January 31, 2009.
This deposit of $100 per person you hope to have attend is refundable until April 15, 2009, when the final payment is required to be sent to the office. At the time of the first payment a break down of males and females is not required. A total number expected to attend is only needed.
The second payment must be post-marked by April 15, 2009. At this time the registration forms for each person attending is necessary. Information from the forms is needed so we can place an order for t-shirts and to order food for the weeks of ministry.
After April 15, 2009, you may still add people to your group at the late fee, and people may be replaced without any financial penalty. However, if after that date you lose attendees and can not replace them, you will receive a refund of the fee paid, minus the $100 non-refundable deposit fee per person.
Materials to be submitted
Each person attending a week of Life Builders Youth Work Camp must have the following on file at the office by April 15, 2009:
-Registration and Liability Forms filled out and signed
-Skills Inventory Sheet completed
-Copy of health insurance card

It is STRONGLY suggested that you make copies of all materials submitted so you have their registration information available in case something happens while on the trip. In fact, making a copy of each person's paper work for each vehicle brought is a great idea. You never know when a person may not still be in the vehicle where their paperwork is held.
When submitting paperwork, we are ok if you print on both sides of the paper.
