How to Register Your Group

Materials to be Submitted

Each person attending a week of Life Builders Youth Work Camp must have the following on file at the office by April 15, 2012:

-Registration and Liability Form and Skills Inventory Sheet filled out and signed:

Teen Paperwork

Adult Paperwork

-Copy of health insurance card

It is STRONGLY suggested that you make copies of all materials submitted so you have their registration information available in case something happens at Life Builders Youth Work Camp. In fact, making a copy of each person's paper work for each vehicle brought is a better idea. You never know when a person may change a vehicle from where their paperwork is held.
